The infection of the cortical and medullary bone can often lead to distinct forms of necrosis, formation of new bone, and obliteration of the bone in its entirety. Cases are usually based on a causative agent, the infections duration, the anatomy of the infected bone, and the path the organisms follow in order to gain passage into the bone. (4)With the increasing cases of osteomyelitis being linked to diabetes, peripheral vascular disease, and the aging population, this disease is becoming more difficult to treat and cure. (1,2,3,4,5,6) Over the past few decades, osteomyelitis has changed its form. Evolving into a disease with many backgrounds, this disease takes heed amongst many polymicrobials. In the past, osteomyelitis cases involved a link to strains of Staphylococcus Aureus. Although Staphylococcus Aureus, also known as S. Aureus, a bacterium, is still the most common organism involved in this disease, a number of infections have been caused by gram negative organisms. These cases would find treatment in surgical debridement and therapy for 4 to 6 weeks of methicillin.(1) Some organisms include mycobacterium, Pseudomonas Aeruginosa, which is found in drug addicts, and organisms that are active in the Gastrointestinal Tract of the elderly. Coagulase-negative staphylococci are pathogens whom also play a distinct role in the presentation of osteomyelitis. (1,4) Pathogenesis   Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Osteomyelitis is caused by microorganisms entering the bone by various mechanisms. These mechanisms include inoculation by distant substances, direct transmission from neighboring soft tissue infection, and hematogenous dissemination via bacteriemia. (3) Ischemia, trauma, and foreign substances can expose vulnerable sites to which bacteria penetrates defenses set by the host. Enzymes are released by a number of phagocytes that attempt to hold the infection. Bacteria can connect tightly to damaged bone. Through interaction with the damaged bone, bacteria withdraws from the host defenses and enters inside the osteoblasts. Furthermore, the infection can lead to sensitive neutrophils, congested blood vessels, and microorganisms which is the basis for acute osteomyelitis.(2) Acute osteomyelitis is an infection that may develop over a short span of time. In children, this osteomyelitis may present as fever, chills, and even pain. The feature that is known to disting uish chronic osteomyelitis is the depletion of living osteocytes also known as necrotic bone.(3) This can occur when osteomyelitis is not properly treated, thus the chronic type of infection may occur.(1) Moreover, two classification systems exist in the Waldvogel and Cierny-Mader Classification in regards to osteomyelitis.   Waldvogel classifies adult osteomyelitis by chronicity and its pathogenesis. These categories of osteomyelitis are labeled hematogenous osteomyelitis, osteomyelitis secondary to a contiguous focus of infection, and chronic osteomyelitis. Acute Hematogenous Osteomyelitis involves a single bone, usually the tibia or femur, and is where bacteria resides in the metaphysis of growing bones. Hematogenous infection in adults is uncommon for the long bones and thus involves the diaphysis. It accounts for 20% of cases of osteomyelitis and is the main cause for children.(3) The common site that affects the adults the most is the vertebrae where hematogenous ostemyeliti s thrives. Patients with this infection tend to have pain in the neck and back. Osteomyelitis secondary to a contiguous focus of infection occurs mostly in adults and accounts for about 80% of all cases.(3) This can occur through bites, wounds and open fractures. The main group of focus here are the individuals who have Peripheral Arterial Disease and Diabetes. This can result in numerous amputations for diabetics. The patient here does not have the proper knowledge to know that the infection is flourishing inside the bone. Trauma and sores can make the foot vulnerable to diabetic neuropathy. In regards to chronic osteomyelitis, about 5% of cases from acute hematogenous osteomyelitis leads to chronic osteomyelitis.(3) Chronic infection tends to have a more contiguous-focus origin than that of a hematogenous osteomyelitis due to the presence of foreign substances inhabiting the infection for a significant amount of time. (2,3) Treatment   Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  The two forms of treatment of osteomyelitis most commonly used are surgical therapy and antibiotic therapy. Surgical therapy is distinguished based on the infections extent. When dealing with acute osteomyelitis, the key component deals with debridement of the dead tissue. In regards to chronic osteomyelitis, there is a resection of foreign substances, and also debridement of soft tissue.(1,2,3,4) Skin and bone grafts may be used to potentiate the start of new bone growth by covering the wound. Some have recommended that increasing the use of particular vitamin supplements, garlic extracts, herbal remedies such as Echinacea, golden seal, and even ginseng may help to relieve certain pains associated with infections. Homeopathic remedies may be useful in counter acting inflammation. (7) Antibiotic-impregnated acrylic beads have also been used for bone infection treatment. The beads should be used along with certain antibiotics. This requires adequate surgical placement in the area specified. Non-biodegradeable beads may be removed surgically after 2-4 weeks. While biodegradeable beads do not require surgical removal and may help for an extended period of time. (3,7) Conclusion   Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Osteomyelitis requires excellent care of injuries and wounds. It may require surgical treatment and antibiotic therapy in order to reduce its effects. Osteomyelitis accounts for a great percentage of cases in adults. Typically, diagnosis of osteomyelitis presents with pain, fever, and other inflammatory signs due to bony infection. This disease has surfaced in Peripheral Arterial Disease and in the small bones of diabetics. Once a bone infection is confirmed by efficient procedures such as biopsy, treatment should follow accordingly. Marco Flores 9/24/12 Utopian Lifestyle Throughout much literature such as Candide, by Voltaire, a concept of a Utopia is introduced. In this book, the utopian society was represented by El Dorado. Here, no realistic world ideals were present, as they were completely satisfied with what they had. They did not pray to God for help or even were curious enough to venture off outside the premises of their city. Lack of curiosity, which is completely against the norm of human nature, was what made them believe they have a perfect society.Most of all, in creating a fictional utopian society, Voltaire is satirizing classical European ways of thought, rather referring to them as the dystopia of the world. Utopias are essentially ideal societies that could never be duplicated in the real world of things. It is literally a fictional way of life that could only be strived for and never be reached. In the novel, they stumble upon El Dorado which is shown to be a magical utopian society. All men a re kind and equal to each other and no one seems to want â€Å"more†, which we as Americans have been accustomed to.They are not seeking to improve themselves in society, which is a concept so baffling for us to understand. It is difficult for us to understand because as a society the motto we live by is â€Å"we want more† and that is why we are far and away from a utopian society. We are never satisfied with what we have, while there in El Dorado, they simply are complacent with where they are. Unlike the citizens of El Dorado, that society was not achievable for Candide and Cacambo. Being an outsider, Candide could not simply drop his ideals to follow those of El Dorado, where gold and gems are just pebbles which they walk upon.Unlike the kids of El Dorado, they could not just play with the precious metals as if they had no value. Not only do their citizens undervalue gems which would make them prosperous throughout the world, but they are satisfied and do not worsh ip nor kill those who do not have the same beliefs as them. They don’t ask God of anything because they believe that what they are provided with is what they could and must use. â€Å"We do not pray to him at all, we have nothing to ask of him; he has given us everything we need; we thank him unceasingly,† 47). When Cacambo and Candide reach Eldorado, Candide concludes that this must be utopia, a place where everything â€Å"is for the best. †Ã‚   However, he is yet unhappy because he misses Cunegonde, and Cacambo has a â€Å"restless spirit† and is not satisfied, either. So, they leave because of their personal desires to achieve more wealth and fame back in Europe. The satire that Voltaire provides on the subject of a Utopia is that there is a great problem the world suffers is that the perfect state is unachievable.Although El Dorado provides a home with no worries or difficulties, Candide always wants more. There is no way to please human beings, even when all of your mistakes are undone and all men are equal. The true problem is that troubles still exist and there is no way to achieve complete happiness. Although El Dorado is seen by Candide as a true Utopia, it is rather the garden which itself is the true Utopia. In the end, they are left with the concept of the personal garden as their true paradise.Thus the theme of the novel is that happiness can only be achieved by concentrating on peace and well-being within one's self, rather than seeking adventures, riches, or material goods. It was the Turk at the conclusion of the novel that describes happiness is like a garden, where you must spend time to cultivate it and let it prosper. Candide realizes that you are the one that must create happiness as things don’t just happen for a reason. As the â€Å"worthy old man seems to have created for himself an existence far preferable to that of the six kings† has done. 93). Throughout his life, Candide has struggled with Pangloss’ philosophy of optimism, while he tries to connect what he’s been taught and what he sees before him. Pangloss always taught him to see that things were always for the best and that there is an explanation for everything. This is difficult for Candide, as he does not see how a world full of war, murder, poverty, rape, and starvation could remotely hold the philosophy that â€Å"everything is for the best in the best of all possible worlds†.On the other side, is Martin with his pessimistic view on the whole world. He sees everyone and everything in a bad light. It is not until the old man where Candide realizes that the world neither is the best or worst of all possibilities. Candide realizes that one can only be content with those around them if they are striving to achieve happiness within, rather than without. Voltaire explains to us that even when one reaches an ideal society where things cannot get more perfect, one is still not satisfied.Rather th an just satirizing the ideas of utopia, he is criticizing our very own human nature. After all attempts to reach bliss through religion and riches, Canide combines the teachings of his two professors, Pangloss and Martin, to the belief in a simpler life full of inner satisfaction where one spends his time cultivating his own garden. So, instead of being a place where no further improvement is necessary, Voltaire explains that a Utopian Life is rather one where you control the variables and focus on your own happiness rather than other variables.

First, my past experience as a child spells clearly on the reasons why I want to become a doctor with specialization in gynecology. The passion for helping other people started early in life when I was still a child. Playing as a doctor was my favorite during those early childhood pleasures and I always enjoyed handling my play patients who were friends, animals and dolls with a lot of love and tenderness. I was also good in handling pregnant women and babies because they seemed to be very delicate and deserving a lot of care. I always had a strong feeling that I could heel my patients because I understood what they were going through. Naturally I  Ã‚   have a liking for life on earth be it for animals or humans; and I would do anything to protect it. I believe that I can make important contributions towards improving the health and medical services of other people in the society. Secondly my personal experiences have also provided impetus in my desire to pursue a medical profession. My own blood brother suffered severe migraines that really affected his early life. He was forced to spend weeks in a darkened room and almost gave up with education. This particular experience really broke my heart as I just wished to bring him back to his normal life. Personal touch with babies in hospitals during medical tours and other events have really influenced my attitude towards sick babies. I have had a baby cousin who suffered from neuroblastoma cancer during infantry and it really broke my heart. The problem started as a lump in her spine that was later diagnosed as a cancer. The cancer culminated into a paralysis on the lower waist. The paralysis had a great impact on her   as evidenced by the walking difficulties she encountered later on. The picture of her on the hospital bed with artificial tubes touched my life and triggered my passion to assist such cases. At local and national hospitals I have participated in future Doctor programs. The tours in these facilities have provided hand-on experience with operations and equipments that have shaped my curiosity to learn more. Successful Doctors have also encouraged me to follow the medical path in the course of our interactions in the hospitals. A terrible first encounter as an ob/gyn has always motivated me into making a difference in the medical profession. This particularly concerns Doctors who had weak skills and showed no empathy to patients. In light of this unfortunate incidence, I have vowed to be different and make good connections with people in the course of helping to address their medical challenges. In my professional career I have a desire to bring significant changes in the provision of medicare for pregnant women and babies. Therefore, the artwork chosen for this reflective essay is the school magazine cover design of SMJK Nan Hwa, Sitiawan for the year 2010. This artwork was produced during the fourteen weeks of internship at SIN BOON BENG PRINTING SDN BHD. As one of the member of SIN BOON BENG PRINTING SDN BHD, I was assigned to do the school magazine cover design for SMJK Nan Hwa, Sitiawan. This is one of the designs that I had given one hundred percent freedom in doing the design based on the clients requirements which means I did the design individually. Every time I start a design, I like to do some brainstorming and mind mapping for further idea generation. Since the school magazines theme is given by the school, I tried to find the denotative and connotative meaning of the theme. Besides, I also did some research on the layout design in order to do a better cover design for the school. I tried to refer to the resources of the company and also some previous design they had did for the school to gain some inspiration. Once I had done with the cover design, I showed it to my supervisor before it is shown to the client. I had done few designs for the school. The final artwork that had chosen is mainly because I was able to create the bond between the design and the theme. My supervisor and mentor had given me some comments for improvements during the design process. This assignment was actually quite challenging. This is because I need to think about the idea and concept all by myself. My supervisor and mentor just gave me some opinions and comments to improve my design. I was not given any guidance to do the design except the clients requirements. In this assignment, I had tried very hard to implement the knowledge that I had learnt in the university. After I did the design, the final artwork was email to the client and the client was quite satisfied about it. Scope of study This study focuses on evaluating and examines the reflecting process on my artwork. This report attempts to present a preview of developments and the involvement in the general area of design process development. Therefore it will address issues such as design methodologies, markets design trend, application of knowledge and others. The development process will be addressed through different features of case studies, design theories and methodologies in this essay. This study summarizes some of the information, as well as information from the previous artworks, in an attempt to illustrate the evolution of my design growing process. It reviews, as well, researches, knowledge and experiences that brought changes in my design career. Objectives/ Aims of essay Self reflection is a very important phase for everyone especially a young designer like me. It is highly beneficial to students development and provides a great value in students graduated life. As the logo design for the school theme is in white colour, I need to consider the suitable colours for the cover. Besides, the client requested to have the oriental feel on the cover and yet can bring up the theme. In order to solve the problems, I did a lot of research on the oriental at first. Who can promise that they have never been selfish throughout their life? Who can say out loud that they never did something that had a bad influence on others, only because of personal benefit? The answer is in the short story â€Å"The Lottery†, which was written by Shirley Jackson in 1948.The story takes place in a small town where everyone has to join the lottery, and the winner will be sacrificed for heavy crops for the town. During the lottery, everyone including Tessie Hutchinson, who wins the lottery, expresses their selfishness both consciously and unconsciously.Shirley Jackson is making a point about selfishness. To be specific, she notes that selfishness is born within mankind and lives deep inside, behind their mask of duplicity. The masks of ‘conscience’ , ‘kindness’ or even ‘love’ will lose their power in front of the selfishness when people are in an extreme situation. The masks cannot cover the true natural sense of mankind. Selfishness is something that no one can reject it.It’s something that everyone owns, but owns without a ny authority to control it. It’s something that is impossible to hide behind the masks of duplicity forever. It’s the basic and strongest devil that born within everyone.But it is possible to change the way of being selfish, by respecting others selfishness, by knowing that other people only care about themselves just like usual, and by finally realize that the easiest way to protect or gain benefit to yourself is by helping others, because devotion will always come back like a boomerang.

In this essay I am going to analyse the dramatic techniques that Arthur Millar uses in his 20th century drama called, ‘A View From The Bridge. ‘ The play is set in Brooklyn in the 1940’s when the Italians were immigrating to America to find work as they were poor and they needed to get away from Italy to find work to feed their families in Italy. Many of these immigrants entered America Illegally. The play is about a skilled docks worker in Brooklyn named Eddie Carbone. Eddie lives with his wife Beatrice and their niece, Catherine. Beatrice’s cousins named Marco and Rodolfo come to America from Italy to find work to feed Marco’s family in Italy, as they are very poor. They plan to enter illegally. Eddie has feelings for his niece Catherine that he should not have. Meanwhile Catherine falls in love with Rodolfo and Eddie hates him because he is jealous as he loves her and he cannot stand any other man looking at her and touching her. As a result Eddie decides to let the Immigration Bureau know that there are illegal immigrants in his building. In the play, Arthur Miller uses a number of dramatic devices many of which are seen in the first act but their significance only becomes apparent to the audience in the second act. These dramatic devices are: use of language, dialogue and its sub textual meanings, stage directions, characters’ actions, props and lighting. The first dramatic technique I am going to analyse is the use of language in Alfieri’s speech at the start of the play. Miller has put this at the start of the play to prepare the audience for what will happen later on in the play. The speech starts to develop tension because there are a lot of negative and violent words used. For example, Alfieri uses terms such as ‘Machine Gun. ‘ The effect of using a term like this is to create tension for the audience, as they now know that something bad is going to happen later on in the play. As the play progresses the audience realise that this is the only time when Alfieri uses negative and violent language. So his words take on a deeper meaning when the audience realises this. I am now going to look at Eddie’s language in the first act. His speech is simple but at the start of the play his words are more vibrant towards Catherine as he tells he that she is â€Å"walking wavy. † At the same time he also refers to her as a â€Å"Madonna. † This gives the audience the idea that she is pure and she is a virgin and Eddie wants her to stay as she is. † Listen, B. , she’ll be with a lot of plumbers? And sailors up and down the street? † This illustrates Eddie’s feelings for Catherine and it can also mean that he does not want any other men looking at Catherine or marrying her. Eddie’s dialogue changes when the cousins arrive. He uses working class American colloquial words, such as â€Å"Yiz have a nice trip? † when he asks the cousins Marco and Rodolfo about their journey. Eddie wants to make it clear that he is the man of the house and that he is in charge. These expressions are only used in the first act when he is speaking to Marco and Rodolfo. This builds tension because the audience know that Eddie is going to start trouble with Marco and Rodolfo later on.

They live in a world of their own and there had never been anything like it and never can be. It is too beautiful altogether, and if they were to set it up it would go to pieces before the first sunset.† (Conrad 10) However, it is because of the women’s purity and naivete that the female characters in the novel–Marlow’s aunt, knitters of black wool, the African mistress, and the Intended–possess a sense of mystery and wield power over the men. The women eventually lead the reader to the discovery of a new truth—not that of the stark reality of the Congo, but of the fact that men yield to women’s will as a way to discover and assert themselves. The women are powerful enough to present the men with a direction, a literal journey, and a sense of purpose. Though Marlow’s aunt and the wool knitters appear for only a short period, their presence precipitates and steers the course of the novel. Marlow’s aunt, who is presented as a disillusioned woman stubbornly adhering to the notion of â€Å"White Man’s Burden,† is the one who actually directs Marlow into his expedition of self-discovery and truth in the first place. This irony is compounded by the fact that it is Marlow’s aunt who comes to the rescue when his own efforts prove fruitless: â€Å"The men said ‘My dear fellow,’ and did nothing. Then–would you believe it?–I tried the women. I, Charlie Marlow, set the women to work–to get a job.† (6) This passage implies that, regardless of Marlow’s condescending views of women, he too realizes (though without admitting it outright) the female influence and his and other men’s powerlessness. It is his aunt’s belief in the fundamental goodness of humanity that gives her power over men; she justifies male imperialistic goals and becomes the object onto which these men project wealth, power, and status. The women in the Belgian company office knit black wool, symbolizing and foreshadowing a sealed fate, dark and tragic. Their power rests in their possession of this fate, and their presence is so domineering that later in the journey, Marlow yields to their unquestionable authority: â€Å"The knitting old woman with the cat obtruded herself upon my memory as a most improper person to be sitting at the other end of such an affair.† (59-60) If Marlow’s aunt is the usher into Darkness, then the knitters are the Darkness’ gatekeepers, and Conrad’s representation of fate as two women is no concidence. The connection between the aunt and the knitters, and eventually the other female characters, binds them in a sisterhood, and their roles only complement their own respective goals in maneuvering the men. The ending of the book is shaped by the African mistress and the Intended. In physical contrast to the ailing Kurtz, the two women are towers (literally, by the descriptions of their height and outstretched arms) of strength, devotion, and purity. Throughout the book, Kurtz is the â€Å"remarkable person† (16), the â€Å"exceptional man† (19), and a quasi-Christ-like figure, but, to Marlow, the Intended is a god: â€Å"bowing my head before the faith that was in her† (70) and â€Å"silencing me into an appalled dumbness† (69). While Kurtz holds truth, the Intended holds illusion, and Marlow’s ultimate lie proves the world of women overcomes the world of truth. It is women’s illusion that shelters men and gives them strength and purpose. This protection can be clearly seen with the Intended: her depiction of Kurtz is drastically different from the reader’s observations, and her distorted image of Kurtz creates his pristine legacy by cle ansing him of his corruption. Her â€Å"inextinguishable light of belief and love† (69) manages to extinguish the darkness of humanity, of the man’s world. Marlow’s asserts women are â€Å"out of it† (44), that they exist in their own ideal space, void of vision and possibility and unbeknownst to truth and reality. Yet Marlow’s journey into the Congo places him into a dreamlike state in which he similarly cannot discern truth from fantasy. The implications of a thick, dark jungle signify a world where â€Å"the reality fades† and â€Å"the inner truth is hidden† (30). Thus, though both the female and male worlds are dark, the female characters dominate because they have not fallen into the male abyss—due to their purity and pledge of responsibility and faith. Marlow’s hazy journey into the Congo and hazy views of the female gender are similar, and this similarity is made even more apparent when he encounters the African mistress, who actually embodies the wilderness itself: â€Å"And in the hush that had fallen suddenly upon the whole sorrowful land, the immense darkness, the colossal bo dy of the fecund and mysterious life seemed to look at her, pensive, and though it had been looking at the image of its own tenebrous and passionate soul.† (56) Ironically, he is strongly attracted to her powerful feminine force, the force of nature, of the female world, which he had once made an effort to avoid. With his travel down the Congo, he has been forced to immerse himself in the female realm, an image of the African mistress with receiving arms, which has similarly â€Å"caressed him [Kurtz]†¦taken him, loved, him, embraced him, got into his veins, consumed his flesh, and sealed his soul†¦Ã¢â‚¬ (44). Marlow’s confused view of women can be read in parallel with Conrad’s own struggle to overtly and covertly balance the strong female presences in his work. In the beginning of the novel, Marlow is disoriented by his aunt, who manages to throw his opinions of gender and power into question. Thereby, Marlow becomes uneasy about his own powerlessness and the fact that women might have an existence aside from his problematic interpretations. In order to adhere to his viewpoints, however, Marlow refuses to admit the nuances he himself allows the reader to observe (i.e. the unmistakable power of his aunt, the knitters, the African mistress, and the Intended beyond his own), and his omission reveals a fear which in turn imparts an independent and potent sphere to those women. It is with this sphere–and the mystery within–that Conrad is able to reveal female power beyond a literal portrayal. That power is deeply psychological and subconscious, and closely intertwi ned amongst the women–the aunt ushering, the knitters guiding, the African mistress embracing, and the Intended cleansing–to conform the male characters to the female will. (Restaurant Owners Face Problems and Crisis, 2010) While there are problems and crises confronting the restaurant business, it is here to stay as it is a part of our daily living. Walsh (2010) also described ten things that make a good restaurant. These include: simplicity, waiters who know the menu, comprehensive menu, no service charge, among others. (Ten Things that Make a Good Restaurant, Feb. 25, 2010) It has been observed that restaurant trends have been continuously evolving around us. â€Å" The top 10 menu trends for 2011 include locally sourced meats and seafood, locally grown produce, sustainability as a culinary theme, nutritious kids’ dishes, hyper-local items, children’s nutrition as a culinary theme, sustainable seafood, gluten-free/food allergy-conscious items, back to basics cuisine and farm-branded ingredients. â€Å"(Food Product Design/ 2011 National Restaurant Trends, Dec. 2010 ) It is worthy to mention that Mayo Clinic’s predictions for 2011 are all about health and flavor such as locally sourced ingredients, small portions, big flavor, new twist on kid friendly and allergy awareness. (Mayo Clinic/Restaurant Trends for 2011, Dec. 2010). Other restaurant trends are predicted by Freeman (2010) who indicates that pies both sweet and savory top the list for 2011 with mini plates, fried vegetables among other trends. (Nation’s Restaurant News/Pies top 2011 restaurant trend list, October 21, 2010).
